A fantastic opportunity has arisen for a skilled and motivated GP Lead Practice Pharmacist to join a leading PCN in South London. The role is designed for experienced Band 8a Pharmacists who are registered with GPhC and possess strong clinical knowledge, documentation skills, and independent prescribing capability.
Job Responsibility:
Running structured medication reviews with a focus on high-risk drug monitoring and long-term conditions such as hypertension, COPD, asthma, and mental health
Independent prescribing to optimise patient outcomes and reduce GP workload
Working collaboratively with GPs, nurses, and multidisciplinary teams
Supporting QOF delivery and PCN targets
Contributing to clinical audits, guideline implementation, and safer prescribing initiatives
Requirements:
GPhC-registered Pharmacist with Independent Prescriber status
Previous experience in a GP surgery or PCN setting
Confidence in running patient-facing clinics autonomously
Ideally enrolled on (or completed) the CPPE Primary Care Pharmacy Education Pathway
Strong clinical knowledge and documentation skills
