Job Description

Evaluates, administers and directs administration of therapeutic procedures utilized in Physical Medicine and Rehabilitation, as prescribed by a referring practitioner. Establishes goals of physical therapy and re-assesses patient's progress at regular, timely intervals, supporting findings with appropriate documentation. This job classification is required to care for patients of all ages. Communicates with patients, families, peers, and other professionals as needed to provide continuity of care.

  • Provide 1:1 or small-group therapy sessions focused on mobility, fall reduction, endurance, and functional tasks needed for safe aging in place within the senior living setting
  • Assist with collecting data for initial and ongoing assessments by interviewing residents, observing movement and function, and reviewing therapy histories and relevant test results as directed by the PT
  • Contribute input to the PT regarding progress toward goals, barriers to participation, and recommendations for updating the plan of care, equipment needs, or level of assistance
  • Participate in interdisciplinary meetings or care conferences to communicate therapy goals, progress, and discharge plans to nursing, wellness staff, and families as requested
