Job Description:

An excellent opportunity has arisen for an enthusiastic and forward-thinking Consultant in Geriatric Medicine to join a progressive Older People's Medicine service in South East England. This substantive consultant post is ideal for candidates passionate about frailty care, community geriatrics, virtual ward development, front-door frailty services, and integrated care pathways for older adults. The department is highly innovative, delivering patient-centred care across hospital and community settings, with strong multidisciplinary collaboration and service development opportunities.

Job Responsibility:

  • Lead consultant ward rounds on the virtual frailty ward
  • Provide senior decision-making for frail older adults in acute and community pathways
  • Deliver Comprehensive Geriatric Assessment across multiple settings
  • Participate in front-door frailty assessments linked to Emergency Department services
  • Support same-day specialist review and admission avoidance pathways
  • Manage rehabilitation, intermediate care, and complex geriatric presentations
  • Provide expert clinical leadership to multidisciplinary teams
  • Work with neighbourhood teams, therapists, GPs, dementia support teams, social care, and voluntary sector partners
  • Support falls pathways, community frailty clinics, and outreach services
  • Contribute to innovative home-based care and expand the virtual ward
  • Supervise junior doctors and multidisciplinary staff
  • Participate in audit, quality improvement, appraisal, and clinical governance
  • Contribute to undergraduate and postgraduate education programmes
  • Support pathway redesign and service transformation

Requirements:

  • Full GMC Registration with a licence to practise
  • Inclusion on the GMC Specialist Register in Geriatric Medicine or within 6 months of CCT/CESR
  • MRCP or equivalent
  • Minimum 1 year NHS experience
  • Ability to manage acute medical emergencies, rehabilitation, and intermediate care
  • Experience working in multidisciplinary teams
  • Teaching and supervision experience
  • Ability to prioritise complex clinical demands
  • Risk management and end-of-life decision-making skills
  • Full driving licence with regular access to a vehicle
  • Higher degree (MD / MSc / PhD / PG Diploma)
