Job Description:

Job Responsibility:

  • Conduct pre-scheduled in-home visits with health plan members
  • Provide 40 hours of availability weekly
  • Execute a strong visit completion rate
  • Conduct Video Visits as needed
  • Maintain training and complete all diagnostic and preventive service (DPS) products as directed

Requirements:

  • Be trained and credentialed for all health plan clients and lines of business in primary coverage state(s)
  • Maintain active state licenses in coverage areas
  • Maintain board certification (as applicable)
  • Maintain multi-state licenses (preferred)
  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format (preferred)
  • Ability to sit, stand, bend, reach, push, pull and lift up to 20 pounds of supplies
  • Accommodate field observations and/or 'ridealong' experiences, as requested

Nice to have:

  • Experience with Signify Health In-Home Health Evaluations, in-home visits, or a similar format
  • Maintain multi-state licenses
