Baptist Health is hiring a Follow Up Care Coordinator to join the BPE Physician Integration team. This is a full-time, hybrid opportunity located at Southbank. The Follow Up Care Coordinator Team has lead responsibility for development and operation of the customer relationship platform used by the department, SalesForce. The team is dedicated to providing assistance to patients post an encounter at a Baptist Free Standing Emergency Department or CareSpot with follow-up care needs and/or finding a family medicine physician. The BH Care Coordinator works to ensure that key relationships between the parties are intact, current and working effectively. Through continuous outreach and relationship building, the coordinator works to inform and educate Baptist Health, Baptist Primary Care, CareSpot and management and clinicians about resources, services, providers and facilities. Additionally, the coordinator provides assistance to consumers with unique health care interests and needs, supporting the New Mover's Program, Corporate Health (PATH) and the Women’s Health Strategy, 4HER program. Additionally, the team provides direct scheduling within EPIC for 19 BPC clinics. Strategic system support is provided by serving as a liaison between patient and clinic offices on strategic service line initiatives including revenue drivers and diagnostic prevention. The Care Coordinator team supports potential and existing patients with a unique, concierge, personalized approach to health care coordination providing exceptional customer service. Team is held to quantitative QA goals to confirm an exceptional customer experience. Metrics include call availability, abandon rate, average handling rate, etc. Monday - Friday 8:00 AM - 5:00 Pm. 1 hour lunch, work from home every Friday after 90 day probationary period if eligible.
