Job Description

CareGivers America NEPA, a Modivcare Personal Care Service, is seeking a Field Supervisor who brings a strong mix of leadership, accountability, and compassion. We’re looking for someone who is hands-on, detail-oriented, and comfortable working in clients’ homes—someone who can identify safety concerns, support caregivers in the field, and lead by example.

Job Responsibility

  • Supervise and support caregivers in clients’ homes to ensure high-quality care
  • Conduct routine home visits, client assessments, and safety inspections
  • Identify and address safety risks within the home environment
  • Monitor and enforce compliance with care plans, company policies, and state regulations
  • Provide hands-on training, coaching, and performance feedback to caregivers
  • Assist with onboarding and orientation of new caregivers
  • Coordinate with care teams to ensure proper staffing and service delivery
  • Address caregiver and client concerns and escalate when needed
  • Maintain accurate documentation of visits and client updates
  • Support quality assurance and compliance initiatives
  • Utilize a company-provided vehicle to travel throughout assigned territories
  • Support outreach efforts by connecting with local providers, care coordinators, and community partners to help grow service

Requirements

  • CNA certification strongly preferred (or equivalent hands-on home care experience)
  • Experience working in home care or directly in patients’ homes
  • Strong ability to recognize safety concerns and ensure a safe care environment
  • Leadership or supervisory experience is a plus
  • Excellent communication and organizational skills
  • Ability to work independently and manage a field-based schedule
  • Comfortable traveling throughout assigned territory
  • Compassionate, patient-focused approach with a commitment to quality care
  • Comfortable training and coaching caregivers in the field
  • Basic technology skills for documentation and scheduling

Nice to have

  • CNA certification strongly preferred
  • Leadership or supervisory experience is a plus
