Job Description

RN Case Manager – Pediatric Home Health. Full-Time RN Case Manager to provide one-on-one pediatric care in the home setting throughout parts of Pike, Wayne, and Susquehanna counties. You will manage care for pediatric patients in their homes, serving as the clinical lead and trusted partner to families.

Job Responsibility

  • Complete comprehensive patient assessments, including OASIS data collection, within established timelines
  • Develop and oversee individualized pediatric care plans in collaboration with physicians and families
  • Provide clinical direction and supervision to LPNs and Home Health Aides
  • Ensure documentation accuracy, timeliness, and regulatory compliance
  • Communicate consistently with families and interdisciplinary care teams
  • Participate in quality improvement initiatives and staff education
  • Maintain a safe, respectful, and supportive care environment

Requirements

  • Active Pennsylvania RN license (Required)
  • BSN preferred
  • 1 year of professional nursing experience preferred
  • Pediatric or home health experience preferred
  • Strong leadership and communication skills
  • Ability to work independently and manage multiple priorities
  • Valid driver’s license
  • Current CPR certification
