Job Description:

Field Care Manager, Registered Nurse position at CVS Health focusing on community-based care management. Join the Community Care team to deliver connected, convenient and compassionate care through best-in-class operating and clinical models. Position involves travel to HCA Healthcare San Antonio and working with members to address full continuum of healthcare and social determinant needs.

Job Responsibility:

Facilitate delivery of appropriate benefits and healthcare information
Develop, implement, and support Health Strategies
Use collaborative process of assessment, planning, facilitation, care coordination
Acts as liaison with member/client/family, employer, provider(s)
Implement and coordinate case management activities for catastrophic cases and chronically ill members
Assess and analyze members/clients medical and/or vocational status
Prepare required documentation of case work activities
Interact and consult with internal multidisciplinary team

Requirements:

Active Texas Compact license
3 years clinical practical experience preference: (diabetes, CHF, CKD, post-acute care, hospice, palliative care, cardiac) with Medicare members
Minimum 2+ years CM, discharge planning and/or home health care coordination experience
Up to 50 to 75% travel to and from HCA Healthcare San Antonio
Associate's degree or Nursing Diploma Required

Ability to occasionally travel within a designated geographic area for in-person case management activities
Excellent analytical and problem-solving skills
Effective communications, organizational, and interpersonal skills
Bachelor's degree
