Playing a vital role in delivering ‘Our Mission’ helping every child with a life-threatening condition live their best life. Based at our hospice, Tŷ Gobaith, in Conwy. The role includes anticipatory grief support, end of life planning and advocacy.
Job Responsibility:
Co-ordinate a small team of Keyworkers in the delivery of a comprehensive pre and post bereavement service
Work alongside the wider teams at Hope House and Tŷ Gobaith
Work with external care providers
Work closely with statutory colleagues in Health, Education and Social Care to ensure the holistic palliative care needs of the family are met
Requirements:
Excels in working as part of a Multi-Professional Team
Ability to confidently communicate in Welsh
Able to work autonomously liaising with parents and supporting families
Effective coordinator to plan interventions and evaluate their effectiveness
Innovative and open to new approaches and ways of working
Confident in acting as an advocate for the child and family
