Job Responsibility

  • Working directly with children, parents and wider family networks in the community
  • Supporting families to improve routines, boundaries, safety and wellbeing
  • Carrying out home visits and delivering targeted practical support
  • Supervising and observing family contact sessions

  • Working closely with social workers and partner agencies
  • Helping families engage positively with services and support plans

Requirements

  • At least 3 years’ experience working with families and children
  • Experience supporting vulnerable families in community or home settings
  • Strong communication and relationship-building skills
  • Confidence working independently and managing visits across the county
  • A full UK driving licence and access to your own vehicle is essential as around 70% of the role involves travelling

  • Prescribe medications based on efficacy, safety, and cost as legally authorized, including medication dosages, routes, and frequencies based on patient characteristics such as age and gender and counsel patients about drug regimens and possible side effects or interactions with other substances such as food supplements, over-the-counter (OTC) medications, or herbal remedies
  • Detect and respond to adverse drug reactions, with special attention to vulnerable populations such as infants, children, pregnant and lactating women, or older adults
  • Educate and engage patients in self-management of acute or chronic illnesses, tailoring instructions to patients' individual circumstances
  • Order, perform, or interpret the results of diagnostic tests such as complete blood counts (CBCs)
  • Maintain timely, complete and detailed records of patients’ personal health information in the electronic medical records system and keep such information confidential in accordance with legal requirements
  • Keep abreast of developments in nursing, medicine and regulatory processes and payer systems such as Medicare, Medicaid, managed care, and private sources, maintaining competence in clinical practice with participation in continuing educational experiences and maintenance of Basic Life Support certification
  • Perform all services in compliance with the most current, accepted professional standards and the standards of the Joint Commission
