This is an excellent opportunity to be part of Cranstoun's Children and Young People's Social Prescribing Service in Dudley, delivering a pilot family social prescribing approach to supporting children and young people aged 11–25 and their families in Dudley and Sandwell. Social prescribing empowers young people to take control of their health and wellbeing through non-clinical, personalised support, focusing on "what matters to me" and connecting them to community activities, services and opportunities. This role is to specifically focus on providing a pilot to test and learn a family social prescribing approach to families identified via the Children and Young Peoples Integrated Neighbourhood Team (CYP INT). As a Social Prescribing Link Worker, you will provide flexible, person-centred support to young people and families, across Dudley and Sandwell, helping them address the social, emotional and practical challenges that impact their wellbeing. You will manage a caseload of children, young people and their families, building strong relationships with partner agencies, including The Dudley Group NHS Foundation Trust, Voluntary, community and social enterprise (VCSE) sector. You will support children, young people and their families to access services and activities that build confidence, improve wellbeing, and reduce isolation. You will be a member of the Children and Young Peoples Integrated Neighbourhood Team.
Job Responsibility
