MD/DO from an accredited institution
active and unrestrictive medical license (or eligibility for licensure by start date)
satisfactory completion of all required due diligence tasks and credentialing
Board Eligible/Board Certified in Internal Medicine by the American Board of Internal Medicine (ABIM), or Family Medicine by the American Board of Family Medicine (ABFM)
residency training in ACGME-accredited Internal Medicine, Family Medicine, Geriatrics, Med-Peds, or Preventive Medicine programs
a minimum of five years of clinical experience
