An exciting opportunity for an enthusiastic, motivated, experienced social worker to join the First Response Team. The team follows a prevent, reduce delay model and ensures a seamless journey through to adult social care, connecting individuals to the right service at the right time to promote and enhance wellbeing. The team is the second point of contact for all new referrals into Adult Social Care, reporting of Safeguarding Adult Concerns, and the Local Authority’s duty for the provision of the AMHP service to coordinate assessments under the Mental Health Act.
Job Responsibility:
Undertake personalised Care Act assessments
Undertake Mental Capacity assessments
Undertake Best Interest decisions
Conduct safeguarding screenings and S42 enquiries
Manage Continuing Health Care processes
Conduct carers assessments
Work within key legislation including Care Act 2014, Mental Capacity Act and Mental Health Act
Apply a strengths-based approach to practice ensuring people’s needs, rights and aspirations remain central to your work
Work in collaboration with individuals, families, carers and partner agencies to achieve holistic outcomes
Requirements:
Registered with Social Work England
Hold a Social work degree/masters
Committed to own professional development
Accountable to people using services, the public, the Council and the regulator (HCPC/ Social Work England)
Ability to converse at ease with customers and provide advice in accurate spoken English
Right to work in the UK (sponsorship not available)
DBS clearance required
Experience of working with adults with a range of complex needs
Strong knowledge of Safeguarding and assessing risk
