Job Responsibility:

  • Helping people to leave hospital and transition back to living at home
  • Visiting Service Users in their own homes to provide care and support as defined in the Care Plan
  • All aspects of personal care, administering medication, monitoring vital signs, exercise and domestic tasks if needed
  • Liaising with medical professionals such as district nurses and pharmacies
  • Rehabilitate and maximise potential, support and enable Service Users in activities to promote independent living and monitor their well-being

Requirements:

  • Access to own vehicle
  • Right to work in the UK (no sponsorship provided)
  • NVQ level 2 or above in Health and Social Care (for higher pay rate)
