Job Description:

The aim is to rehabilitate and maximise potential, to support and enable Service Users in a range of activities to promote independent living and monitor their well-being.

Job Responsibility:

Helping people to leave hospital and transition back home
Visiting Service Users in their own homes to provide care and support as per Care Plan
Providing personal care, administering medication, monitoring vital signs, exercise and domestic tasks
Liaising with medical professionals such as district nurses and pharmacies
Rehabilitating and maximising potential to promote independent living
Monitoring well-being

Requirements:

Access to own vehicle
Right to work in the UK
NVQ level 2 or above in Health and Social Care (for higher pay rate)
