Job Description:

Part-time (18.5 hours) Enablement Coordinator position in the Adult Social Care Enablement Service in Gloucester. The Enablement Service supports vulnerable adults who may have a disability and challenging home circumstances, aiming to develop daily living skills and promote greater control over people’s lives. Support work takes place in people’s homes, community, and in Drop In centres based in libraries.

Job Responsibility:

  • Work directly with adults across Gloucestershire, supporting them to maintain or regain independence, improve wellbeing, and access appropriate services
  • combine enablement and care navigation functions, focusing on early intervention, crisis support, and community connection
  • work collaboratively with individuals, families, carers, and professionals to deliver person-centred, strengths-based support
  • prioritise and manage own time confidently
  • develop and maintain good working relationships with outside partnerships to signpost individuals as required

Requirements:

  • Level 3 Qualification in Health and Social Care, Community Care or equivalent qualification or experience
  • training related to Health and Social Care
  • experience of working with individuals who need support to achieve outcomes
  • evidence of good interpersonal skills and good verbal / written communication
  • good IT skills, specifically Microsoft Office and an ability to use and learn in-house systems
