Job Description

Hello Neighbor is seeking a dynamic Case Manager who will join our Employment Team in supporting refugees and other qualifying participants enrolled in our Refugee Social Services program. The Case Manager will complement the work of the RSS Employment Coordinator by providing a range of additional supportive services that clients and their families may need, including assessing barriers to self-sufficiency, developing service plans, making referrals, coordinating appointments for services, teaching critical life skills, and assisting with the job search process including interviews and onboarding when needed. In addition to the RSS programming, the Case Manager will also assist with other departmental needs and programs. This position is largely a community-based role. The Case Manager will assist clients to meet their basic needs and build skills needed for independence through a range of individualized supportive services, both directly and through engagements with other programs and service providers and assorted healthcare, housing, education, and community-based service providers.

Job Responsibility

  • Alongside the Employment Coordinator, the Case Manager is responsible for the overall management of the case unit, including screening for program eligibility, completing an intake assessment, developing Family Self-sufficiency Plans (FSSP), providing required services, and completing and maintaining all required documentation
  • Provide Other Case Management services to families of clients enrolled in Hello Neighbor’s employment programs
  • Transport and assist clients to navigate a variety of appointments, including but not limited to applications for documents or public assistance, medical appointments, school enrollments, and immigration appointments
  • Coordinate referrals to other Hello Neighbor programs as well as outside services as needed for RSS clients (i.e., medical, employment, ESL, mental health)
  • Arrange for and use interpretation and translation services to ensure effective communication with clients
  • Record case notes, complete reports, and maintain all required documentation accurately and in a timely manner
  • Leverage the assistance of Hello Neighbor volunteers in coordination with the Volunteer and Community Engagement Manager in order to assist RSS clients
  • Network and develop community contacts with other social service providers, including both mainstream providers and those that serve refugees exclusively
  • Provide rotating support for office hours held on Wednesdays 2-6 p.m.
  • Support the Employment department at large as needed including job training, resume assistance, client enrollments and closures etc.

Requirements

  • Degree in Social Work or a related field preferred
  • Relevant work experience and a demonstrated ability to effectively work with refugee and immigrant communities
  • Alignment with and support of Hello Neighbor’s core values
  • Flexibility to work some evening and weekend hours
  • Personal vehicle, valid driver’s license, and proof of insurance that meets Hello Neighbor’s insurance requirements
  • Clearances required: Pennsylvania Child Abuse History Clearance, Pennsylvania State Police Criminal History Clearance, Federal Bureau of Investigations (FBI) Criminal History Clearance (PA Act 23 & Act 24)
  • Mandatory Reporter and/or PSEA training may also be required depending on job requirements
  • The successful candidate must be currently authorized to work in the United States
