This is a new role created to further improve our support in respect of children and young people with SEND. The role will ensure that the social care needs of children and young people are accurately reflected in their Education Health and Care needs assessments and plans and that families are aware of the support and services available to them.
Job Responsibility:
Ensure the social care needs of children and young people are accurately reflected in their Education Health and Care needs assessments and plans
Ensure families are aware of the support and services available to them
Contact families directly to gather relevant information, provide advice and guidance
Make contact with other services on behalf of families where appropriate
Be up to date regarding services and support in the area
Contribute to service development
Requirements:
Experience of working directly with children and families
Educated to Level 3 standard with 5 GCSEs or equivalent including English and Maths at Grade C or above
Knowledge and understanding of children and young people with Special Education Needs and Disability
Excellent communication skills
