Job Description:

Hackney Council is seeking an Early Help Family Support Practitioner to join their Early Help service, providing intensive support to families with multiple and complex needs.

Job Responsibility:

  • Deliver child-centred family assessments using evidence-based tools
  • Provide targeted support to children and families in the home and community
  • Act as Lead Professional coordinating support through MAT and Team Around the Child meetings
  • Develop Early Help plans with clear outcomes and review targets
  • Deliver parenting support and parenting groups to strengthen parenting capacity and improve child outcomes
  • Work in line with safeguarding procedures and Hackney’s Family Support Practice Guidance

Requirements:

  • Level 3 qualification in Childcare & Education, Primary Education, Health, Social Work, or working with parents
  • Significant experience working with children and families
  • Experience delivering parenting support or programmes to vulnerable families
  • Experience providing advice, guidance, and home-based interventions
  • Strong child-centred and outcomes-focused approach
