The Discharge Coordinator, in collaboration with providers and nursing staff, is responsible for coordinating all aspects of the discharge process starting on admission for specific patient populations.
Job Responsibility:
Coordinating all aspects of the discharge process starting on admission for specific patient populations
Creation of individualized patient and family education plans for discharge
Coordination of teaching plans for patients and families
Follow-up and coordination with nursing and the multidisciplinary team
Collaboration with case management for home care, durable medical equipment, discharge medications, and care needs
Requirements:
Associate's Degree - NICU
Associate's Degree in Nursing while pursuing a Bachelor's Degree in Nursing will be accepted - all departments except NICU
1 Year of Experience
BLS for Health Providers (American Heart Association)
CA RN License
PALS or PEARS (American Heart Association) - within 6 months of hire
