We’re excited to offer a full-time Relationship Manager (Disability Case Manager) opportunity at Hearth Support Services. Based in our Glen Iris office, this role is all about making a genuine difference. You’ll provide compassionate, personalised support to people with disabilities, building trusted relationships, empowering them to live independently, pursue their goals, and truly live the life they want.

Job Responsibility

  • Develop and maintain positive relationships with clients, their families, and other stakeholders to understand their unique needs and preferences
  • Conduct comprehensive assessments to evaluate clients' physical, emotional, and social needs
  • Collaborate with clients to develop individualised support plans and coordinate the delivery of various disability services
  • Advocate on behalf of clients to ensure they receive the appropriate level of funding and access to relevant services
  • Monitor and review client progress, adjusting support plans as needed
  • Maintain detailed client records and comply with all relevant policies and procedures
  • Contribute to the continuous improvement of our disability support services

Requirements

  • Relevant tertiary qualification in social work, disability studies, or a related field
  • Minimum 2 years' experience working in the disability sector, preferably as a case manager or support coordinator
  • Excellent communication and interpersonal skills, with the ability to build trust and rapport with clients
  • Strong problem-solving and decision-making skills, with a focus on person-centred, outcomes-based support
  • Familiarity with the National Disability Insurance Scheme (NDIS) and other relevant government programs and legislation
  • Flexible and adaptable, with the ability to work independently and as part of a team
  • Empathetic, compassionate, and committed to making a positive difference in the lives of people with disabilities
