Job Description:

This strategic and hands-on leadership role will drive Medicare ACO performance and value-based care initiatives across a growing national network of nurse practitioner-owned practices. This individual will operate at the intersection of population health strategy, payer performance, clinical transformation, and data-driven execution, helping scale a high-impact care delivery model that improves outcomes for patients and communities.

Job Responsibility:

  • Lead day-to-day oversight and performance of Medicare ACO programs, ensuring strong outcomes across quality, utilization, and financial metrics
  • Drive performance across Medicare Advantage, Medicaid, and commercial value-based care contracts
  • Monitor and communicate contract performance trends, risks, and opportunities to stakeholders and leadership
  • Develop and execute strategies to improve cost performance, quality scores, and shared savings outcomes
  • Partner with primary care practices to support care model transformation, workflow optimization, and population health initiatives
  • Translate complex data into actionable insights for clinicians and practice teams
  • Collaborate with product and data teams to define key metrics, dashboards, and reporting tools
  • Identify performance gaps, test interventions, and iterate quickly using data-driven approaches
  • Manage and mentor a team of analysts and practice transformation specialists while contributing individually
  • Build strong cross-functional partnerships across clinical, operations, product, data, and finance teams
  • Engage with payer partners to support contract success and program evolution
  • Contribute to scaling value-based care programs nationally as the network expands

Requirements:

  • 7+ years of experience in value-based care, population health, Medicare ACOs, or risk-based contracting
  • Direct experience managing or operating Medicare ACO programs and/or Medicare risk arrangements
  • Strong understanding of value-based care performance drivers, including quality measures, utilization, and shared savings models
  • Experience working closely with primary care practices and clinical teams
  • Proven ability to lead, mentor, and develop high-performing teams
  • Strong analytical skills with the ability to translate data into actionable strategies
  • Experience collaborating with product and data teams in healthcare or health tech environments
  • Comfort operating in a fast-paced, high-growth or startup environment
  • Excellent communication skills with the ability to influence diverse stakeholders
  • Bachelor’s degree required
