The Director, Home Care Clinical Services (DCS), is responsible for the planning, direction, coordination, supervision, evaluation and provision of all nursing services provided by the assigned facility location. The DCS oversees the coordination, continuity, and quality of clinical care, as well as the management of patient services and monitors, evaluates, and takes action on clinical measures of performance. The DCS participates in the recruitment of associates, and provides the supervision necessary to facilitate the provision of quality care and patient satisfaction; and leads the education, training, and development of associates.

Providing supervision to facilitate quality care and patient satisfaction

Directing and managing clinical operations
Serving as a clinical mentor and coach
Providing strategic leadership in staffing, productivity, and performance optimization
Acting as a clinical liaison for interdisciplinary teams, patients, and families
Recruiting, training, and retaining top clinical talent
Setting clear expectations and performance goals
Delivering regular coaching and feedback
Cultivating a positive work environment
Leading educational initiatives including orientation and in-service training
Ensuring compliance with state, federal, and accreditation standards (ACHC)
Monitoring and improving clinical outcomes using data tools
Overseeing accurate documentation, OASIS scoring, and physician order management
Implementing and leading QAPI programs
Ensuring individualized care planning
Conducting supervisory visits and supporting clinical fieldwork
Maintaining continuity of care
Participating in budget planning, financial reviews, and strategic initiatives
Supporting regulatory readiness
Providing operational backup to the Director of Operations
Requirements:
Current RN, and a state license in good standing
Minimum 3 years of clinical leadership experience
At least 2 years of home health experience
Strong knowledge of regulatory compliance, OASIS, and outcome-based care models
Excellent interpersonal, organizational, and leadership skills
