Job Responsibility:

  • Develops, coaches and serves as a resource and liaison to the Admissions Team and all staff who assist in the process from referral inquiry to admission
  • Ensure all patient and family services offered by the Hospice, during the referral inquiry to admission process, comply with federal, state(s) and local regulatory requirements
  • Assume responsibility for a patient and family including, assessing, planning, implementing and evaluating care, as assigned
  • When necessary, communicate with attending physicians, other hospice staff members and other agencies as needed to coordinate optimal care and use of resources for the patient and families
  • Evaluate staff productivity, referral and admission data on a routine basis to adequate staffing exists to support excellence in response to referral inquiry and admissions and meet budgetary requirements
  • Monitor Vital Signs Respiratory Rate report to track referrals and admission data to coordinate related follow up with the Executive Director (ED), Director-Business Development (DBD) and leadership, as required
  • Mentor and model follow-up activities on active and pending referrals to meet defined conversion rate goals on a consistent basis
  • Participate in site leadership team meetings: Collaborate to address operational issues
  • Participate in short- and long-term strategic planning
  • Meet with the Admissions Team weekly to review referral and admissions data, pending referral status, provide follow up from leadership meetings and provide ongoing support and education
  • Participate in monthly Director-Clinical Operations (DCO) and Team Director (TD) meetings for operations feedback and ongoing education

Requirements:

  • Bachelor’s degree
  • 4+ years previous hospice, home health or case management experience, or equivalent
  • Demonstrated ability to effectively direct a variety of personnel and utilize mentoring and motivational skills
  • Licensed to practice as a registered nurse in the state of agency operation
  • Must be a licensed driver who can travel to all business locations
  • current Texas RN License Required
