Job Description

The Dietitian is a vital member of the interdisciplinary team within the PACE program. This role is responsible for providing comprehensive nutritional assessments, individualized care planning, and ongoing support to meet the unique dietary needs of frail and elderly participants. The Dietitian develops and manages therapeutic diets, educates participants and caregivers, and ensures safe and appropriate food service practices to promote health, independence, and quality of life.

Job Responsibility

  • Conduct comprehensive nutritional assessments for new and current PACE participants
  • Develop, implement, and monitor individualized nutrition care plans in collaboration with the interdisciplinary team
  • Provide nutrition education and counseling to participants, caregivers, and staff
  • Oversee therapeutic diets, dietary modifications, and supplement use
  • Monitor food service operations including sanitation, meal compliance, and tray accuracy
  • Document nutrition notes and maintain accurate records in the electronic medical record
  • Participate in interdisciplinary team meetings, Member Council, and Nutritional Improvement Program (NIP) reviews
  • Assist with training dietary and clinical staff, and contribute to policy and procedure development
  • Perform home and facility visits as needed to support participant dietary needs
  • Maintain continuing education requirements for licensure and professional registration

Requirements

  • Bachelor’s degree (or higher) in food and nutrition or dietetics from an accredited institution
  • Current registration with the American Dietetic Association and state certification required
  • Minimum of two (2) years’ experience working with frail or elderly populations preferred
  • Experience working on a multi-disciplinary team in hospital, nursing home, or community-based settings strongly preferred
  • Valid driver’s license, proof of insurance, and reliable transportation required
  • Medical clearance for communicable diseases and up-to-date immunizations required prior to direct participant contact

Nice to have

  • Minimum of two (2) years’ experience working with frail or elderly populations preferred
  • Experience working on a multi-disciplinary team in hospital, nursing home, or community-based settings strongly preferred
