An opportunity has arisen for a full time Band 4 Dietetic Assistant post. The successful candidate will be working with the Community Adults Nutrition & Dietetic Service. You will be based at Sherwood Rise health Centre and will be expected to deliver community clinics across various health Centres in Nottingham City. The post is both interesting and varied with a particular focus on working with malnourished patients requiring Oral Nutritional Supplements (ONS).
Job Responsibility:
To support the Nutrition and Dietetic Services in the community including care homes and home visits
To support the Dietitians with the Oral Nutritional Supplement service (ONS)
To support Dietitians with the delivery of patient group education sessions
To support Dietitians with the delivery of nutrition related topics to other professionals
To support the Community Diabetes team with Type 2 diabetes structured education
To undertake individual treatment and interventions with clients under the supervision of qualified staff
To support qualified staff with providing therapeutic dietary and lifestyle advice to patients
To support qualified staff in the Oral Nutritional Supplements (ONS) service by regularly monitoring waiting lists and carrying out patient reviews
To monitor individual patients nutritional intakes and requirements through the evaluation of a complex range of information
To utilise the following skills and competencies for monitoring adult patients
To communicate relevant dietary advice and lifestyle advice effectively
To support qualified staff to provide nutrition and dietetic services to care homes
To support qualified staff to provide a domiciliary service to housebound patients and their carers to review patients
To accept responsibility for designated caseload
To refer service users to external agencies and specialists when their needs are beyond the assistant practitioner (AP) role
To have the ability to gain valid informed consent and to identify through assessment, clients who lack capacity
To attend and contribute to team meetings and case discussions using effective communication
To maintain comprehensive and accurate records in line with polices
To participate in the development of good practice for the AP role within Nottingham CityCare Partnership
To provide support to other Assistant Practitioners as appropriate
To be responsible for own Continuing Professional Development
To actively participate in the services training strategy
To use information technology daily and participate in identified data collection and statistics
To actively promote equality and diversity within service delivery and respect peoples individual rights
To ensure knowledge, skills and competencies are kept up to date
To be involved in organisational appraisal, clinical and management supervision
To assist in the training and development of others
To support the delivery of patient group education sessions
To support the delivery of evidence-based training on diet and nutrition-related topics
To undertake any other duties that may be reasonably required by clinicians
To actively participate in audit and research
Supporting the clinical dietetic team to work with CityCares neighbourhood teams and other professional groups
Requirements:
Experience of working in a multi-disciplinary team
Experience of working with individuals who have long-term conditions
Experience of record keeping and accurate data recording
NVQ /QCF 3 in Health and Social care or equivalent qualification
To have completed or willing to work towards a health professional Foundation Degree
Knowledge of Health and Social Care provision
Knowledge of a range of communication skills to gain co-operation with treatment where there may be significant barriers to understanding or ability to co-operate with the treatment
Knowledge of need and risk in relation to Health Care provision in the community
Understanding the concepts of assessment and care planning
Awareness of promoting/enabling independence and self-care
Effective communication skills (Verbal and written) and good interpersonal skills
Ability to prioritise and plan workload
Ability to follow the Health Care Support Workers code of conduct policy and maintain professional boundaries
Ability to recognise the need for help and support from team members or other professionals
Ability to contribute to the effective working of a team
Ability to involve individuals in a person-centred approach to care
Negotiation skills

Full driving licence and the ability to travel between locations
