Job Description

A rewarding opportunity to be involved in a Dementia Centre (DC) initiative to support people living with dementia who are transitioning from hospital settings back to their homes or into residential care. Job Responsibility

  • Facilitate on the ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff
  • Work collaboratively with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care
  • Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required

Requirements

  • Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands
  • The ability to work both independently and as part of a team
  • Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure the consistent provision of exceptional service to internal and external customers
  • A current driver’s licence and safe driving record
  • Willingness to travel and work intrastate and interstate to support the delivery of DC services
  • Qualification in relevant field
  • Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable
