Job Responsibility

Facilitate on the ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff
Work collaboratively with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care
Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required
Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required
