Job Description:

The Extracare Charitable Trust are recruiting for a Dementia and Mental Wellbeing Enabler to join our friendly and supportive team at Earlsdon Park Village, based in Coventry. We are excited to be recruiting for a Dementia and Mental Wellbeing Enabler to support the village’s commitment in delivering ExtraCare’s mission of “better lives for older people”.

Job Responsibility:

  • Provide one to one support to older adults living with dementia and common mental health conditions
  • Working with a multidisciplinary team and being first point of contact when required
  • Responsible for completing cognitive assessments, mood assessments, Mental Capacity Assessments and Best Interest Meetings
  • Make sound well-informed decisions while balancing the needs of the residents
  • Complete risk assessments, and where required raise safeguarding concerns

Requirements:

  • Proven experience in providing support to older adults, particularly those living with Dementia and Mental health conditions
  • Exceptional communication and interpersonal skills, with the ability to establish rapport with residents, their families and external professionals
  • Ability to manage an independent caseload for one-to-one work, deliverance of group interventions, and build connections with external professionals
  • Respond to changing needs, adapt to new situations, and make quick decisions regarding a residents’ wellbeing
