Job Description:

Baptist Health is hiring a Coordinator for the Baptist Health Wellness Centers. This is a full-time, in-person opportunity to join the Baptist Health Partnerships team as the new onsite Referral Coordinator and Wellness Coach. Baptist Health Wellness Centers are located in Riverside, Northside, Mandarin, Nocatee and Fleming Island. This position will float to all locations.

Job Responsibility:

  • Processing the referrals received to the Baptist Health Wellness Centers including scheduling appointments in a timely manner, maintaining detailed records, clear communication with all parties, and navigating patient questions
  • Confidently learning and working within multiple technology systems such as: Microsoft Office programs [Outlook, Word, Excel, PowerPoint, etc.], Epic EMR system, and various web-based platforms like Eventbrite, SECA, Converge Virtual Merchant, and Smartsheet
  • Performing Wellness Coaching sessions delivering quality health education guidance and administering baseline health screenings
  • Accurately documenting wellness coaching session information and program participation data
  • Supporting colleagues with facility operations and the daily tasks of opening/closing the office, maintaining an organized space & supply needs, greeting and providing assistance to all patrons, and preparing the Center for scheduled programming
  • Assisting patrons with healthcare navigation questions and needs
  • Program coordination including the development, promotion, implementation and evaluation of health education events

Requirements:

  • Bachelor's Degree
  • 1-2 years Experience coaching wellness needs
  • 1-2 years Experience with evaluating wellness and health promotion programs
  • 1-2 years Experience implementing wellness and health promotion programs
  • 1-2 years Experience conducting health risk assessments
  • Basic Life Support (BLS) Required
  • Health Coach Certification Required
  • Cardiopulmonary Resuscitation Certification (CPR) Required
  • Wellness Coach Certification required or completed within 90 days
