You’ll work closely with our Registered Manager, Clinical Team and external professionals to coordinate person-centred care that truly makes an impact.
Job Responsibility:
Coordinating complex care packages and allocating care workers to People We Support (PWS) based on their individual clinical and personal needs
Maintaining detailed care schedules and ensuring staff have access to accurate, up-to-date care plans
Developing and regularly reviewing person-centred care plans in collaboration with families and healthcare professionals
Ensuring care plans reflect key areas including medication management, mobility support, nutrition and emotional wellbeing
Acting as a key point of contact for service users, families, healthcare professionals and funding authorities
Attending safeguarding, risk management and care review meetings related to service user care
Ensuring services remain compliant with CQC standards and maintaining accurate care records
Supporting care staff with guidance, communication and oversight to ensure consistent, high-quality care delivery
Contributing to audits, client feedback reviews and continuous service improvement initiatives
Requirements:
Previous experience (minimum one year) in care coordination, within the Community Complex Care sector
Strong organisation and communication skills
A proactive, solution-focused mindset
Ability to remain professional in challenging situations
Understanding of CQC standards and regulatory compliance
