Job Description:

This is a rewarding and varied role where no two days are the same. You’ll play a vital part in ensuring our services are safely staffed, well-organised and delivered to the highest standards. You’ll work closely with our Registered Manager, Clinical Team and external professionals to coordinate person-centred care that truly makes an impact.

Job Responsibility:

  • Coordinating complex care packages and allocating care workers to People We Support (PWS) based on their individual clinical and personal needs
  • Maintaining detailed care schedules and ensuring staff have access to accurate, up-to-date care plans
  • Developing and regularly reviewing person-centred care plans in collaboration with families and healthcare professionals
  • Ensuring care plans reflect key areas including medication management, mobility support, nutrition and emotional wellbeing
  • Acting as a key point of contact for service users, families, healthcare professionals and funding authorities
  • Attending safeguarding, risk management and care review meetings related to service user care
  • Ensuring services remain compliant with CQC standards and maintaining accurate care records
  • Supporting care staff with guidance, communication and oversight to ensure consistent, high-quality care delivery
  • Contributing to audits, client feedback reviews and continuous service improvement initiatives

Requirements:

  • Previous experience (minimum one year) in care coordination, within the Community Complex Care sector
  • Strong organisation and communication skills
  • A proactive, solution-focused mindset
  • Ability to remain professional in challenging situations
  • Understanding of CQC standards and regulatory compliance
