Job Description

This role coordinates, manages and develops community based supports for individuals with developmental disabilities in both Erie and Chautauqua counties. Job Responsibility

  • Coordinates, manages and develops community based supports for individuals with developmental disabilities in both Erie and Chautauqua counties
  • Oversite and monitoring of community based services, including development and implementation of customized support plans, person centered reviews and associated documentation of services

Requirements

  • Associate’s Degree in Human Services or related field
  • Five (5) years experience in supervisory and/or coordinating activities in a human service agency
  • Valid NYS drivers license
