Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency. Identify and address the strengths, which will aid the individual or family in the recovery and resiliency process, as well as the barriers that could impede the development of skills needed for independent functioning in the community. Support the individual and the family in crisis situations; provide personalized interventions to develop, facilitate, or enhance an individual’s ability to make informed and independent decisions. The patient care assignment may include children who are at risk of/or experiencing serious emotional, neurobiological, and/or behavioral disorders; adults with severe mental illness (SMI), individuals with chronic substance abuse, and/or individuals with co-occurring disorders. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups.
Job Responsibility:
Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency
Identify and address strengths and barriers to independent functioning
Support individuals and families in crisis situations
Provide personalized interventions to enhance decision-making
Work with patients across various age groups and conditions (children with emotional/behavioral disorders, adults with SMI, substance abuse, co-occurring disorders)
Adhere to Patient Centered Medical Home/Specialty Practice expectations
Work to eliminate barriers to culturally competent medical services
Assist patients to access community resources and financial assistance
Collaborate with medical providers and multidisciplinary teams
Facilitate development of life skills (daily living, interpersonal coping, socialization, community functioning)
Plan for professional growth
Provide wellness information on mental conditions, medication, symptom management
Assist in development of service plans, crisis management plans
Assist in obtaining rehabilitative, medical, financial, housing, and judicial support services
Encourage development of natural supports
Conduct assessments of strengths, needs, and barriers
Provide crisis intervention and assess for early signs of relapse
Coordinate and monitor service use, track client activities
Document all client encounters and maintain files
Maintain and report program statistics
Follow patient safety protocols and encourage patient participation in safety
Requirements:
Bachelor's Degree in Related Discipline
1 year directly related experience
CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
Valid New Mexico Driver's License
UNM Vehicle Operator's Permit w/in 60 days
