Community Support Worker

• Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency
• Identify and address strengths and barriers to independent functioning
• Support individuals and families in crisis situations
• Provide personalized interventions to enhance decision-making
• Work with children at risk, adults with severe mental illness, individuals with substance abuse, and co-occurring disorders
• Adhere to Patient Centered Medical Home expectations
• Work to eliminate barriers to culturally competent medical services
• Assist patients to access community resources and financial assistance
• Collaborate with medical providers and multidisciplinary teams
• Facilitate development of life skills
• Plan for professional growth
• Provide wellness information and resources
• Assist in development of service plans
• Assist client in obtaining access to medical and rehabilitative services
• Encourage development of natural supports
• Conduct assessments
• Provide crisis intervention
• Coordinate and monitor use of services
• Document all client encounters
• Maintain and report statistics

• Bachelor's Degree in Related Discipline
• 1 year directly related experience
• CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days

• Bilingual English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
• Certified Psychosocial Rehabilitation Practitioner
• Licensed Alcohol and Drug Abuse Counselor or LASI eligible

Receive 17% Weekday Nights, 26% Weekend Nights and 15% Weekend Day shift differentials
