Job Description:

Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency. Identify and address the strengths, which will aid the individual or family in the recovery and resiliency process, as well as the barriers that could impede the development of skills needed for independent functioning in the community. Support the individual and the family in crisis situations; provide personalized interventions to develop, facilitate, or enhance an individual’s ability to make informed and independent decisions. The patient care assignment may include children who are at risk of/or experiencing serious emotional, neurobiological, and/or behavioral disorders; adults with severe mental illness (SMI), individuals with chronic substance abuse, and/or individuals with co-occurring disorders. Ensure adherence to Hospitals and departmental policies and procedures. Patient care assignment may include neonate, pediatric, adolescent, adult, and geriatric age groups.

Job Responsibility:

  • Coordinate and provide services and resources to individuals/families necessary to promote rehabilitation, recovery, and resiliency
  • Identify and address the strengths and barriers in the recovery process
  • Support the individual and the family in crisis situations
  • Provide personalized interventions to enhance decision-making
  • Work with children at risk, adults with severe mental illness, individuals with substance abuse, and co-occurring disorders
  • Adhere to core expectations of the Patient Centered Medical Home/Specialty Practice
  • Work to eliminate barriers to access culturally competent medical services
  • Collaborate with medical providers and community resources
  • Facilitate development of life skills in daily living, interpersonal coping, socialization, and community functioning
  • Plan for professional growth
  • Provide wellness information on mental conditions, medication, and relapse prevention
  • Assist in development of service plans including crisis management
  • Assist in obtaining access to rehabilitative, medical, and other services including housing and benefits
  • Encourage development of natural supports
  • Conduct assessments of strengths, needs, and barriers
  • Assess, support and intervene in crisis situations
  • Coordinate and monitor use of services and track client activities
  • Document all client encounters and maintain files
  • Maintain and report applicable statistics
  • Follow patient safety-related policies and procedures
  • Encourage patients to actively participate in their own care

Requirements:

  • Bachelor's Degree in Related Discipline
  • 1 year directly related experience
  • CPR for Healthcare/BLS Prov or Prof Rescuers w/in 30 days
  • Valid New Mexico Driver's License
  • UNM Vehicle Operator's Permit w/in 60 days

Nice to have:

  • Bilingual English, Spanish, Keres, Tewa, Tiwa, Towa, Zuni, or Navajo
  • Certified Psychosocial Rehabilitation Practitioner
  • Licensed Alcohol and Drug Abuse Counselor or LASI eligible
