Relevant Tertiary qualifications in Community Services, Mental Health and Disability/and or Similar
Preferably 1+ year of experience in a similar role
Demonstrated experience with Facilitating Multi-Disciplinary Coordinated Planning and Develop Collaborative Relationships and Networks between Service Providers
Extensive knowledge of Mainstream and NDIS systems and Practices

Working with participants strengthens their ability to coordinate their support to live their best lives
Supporting participants strengthen their capacity to establish their network and maintain support services, links, and relationships
Supporting participants to understand their NDIS plan, goals, and objectives whilst promoting choice and control
Preparing for Plan Reviews and Attending with the Participant, Family, or Representative
