CityCare is a social Enterprise providing healthcare for the NHS with any profits re-invested back into the local community. We are looking for Band 5 community nurse who can be responsible for delivering a high standard of nursing care to patients in a community setting.
Job Responsibility:
To provide nursing care to a variety of patients in their own home
To work as part of the integrated health and social care teams in the community, providing holistic care to patients
To provide holistic and individualised care to patients and carers as working autonomously and as part of a team
To regularly take responsibility for care and workload management in the absence of the Team Leader or deputy
To provide nursing care for house bound patients who are registered with a Nottingham City GP
To participate actively in the services training strategy
To participate in continuing professional development as relevant to maintain NMC registration requirements
To work across the Nottingham City Community Nursing Teams as required
To supervise and support student nurses on placements with the community nursing teams
To supervise unregistered nurses who have been delegated patient care
Contribute to the public health agenda by raising awareness of health and well-being needs of individuals
Plan, deliver and evaluate programmes of care to address peoples health and well-being needs
Deliver evidence-based care
Seeks patient and carer feedback about the service
Deliver a high standard of care to patients and carers
Contribute to the continual improvement of the service
Maintain a professional Standers as outlined in NMC code
Have an awareness of Clinical Governance agenda and apply relevant risk management systems
Participate in relevant research and audit activity
Regularly take responsibility for the appropriate delegation of nursing care to team members
Promote and maintain record taking practice in line with the CityCare and NMC and professional standards
Liaise with individuals and groups in a wide variety of settings and situations
Communication and escalate as required
Good communication and team working
To be responsible for the development of own knowledge and clinical practice and recognises ones own limitations
Act as a mentor to a variety of nursing students
Participate in the development of other team members
Participate in the supervision framework as a supervisee and supervisor
Maintain a portfolio to demonstrate continuing professional development
Contribute to the implementation of service developments
Participate in the Personal Development process and take responsibility for reaching agreed development objectives
Attend mandatory training
Utilise enhanced knowledge and skills to promote continuous service improvements
Promote and maintain best practice that ensures the health safety and security of self, patients and colleagues
Practice in accordance with Safeguarding legislation and guidelines
In the absence of District Nurse Team Leader or deputy takes responsibility for the needs of patients and carers on the community nursing caseload and ensures that appropriate delegation and supervision of work is carried out
Requirements:
Ability to be flexible over hours worked within contracted hours to meet the needs of the service
Ability to work out of hours
Full driving licence and the ability to travel between locations
To participate in and fulfil the requirements of the Directors on-call arrangements
Up to date clinical knowledge including wound care, management of continence and palliative care

Either post registration experience or completion of a community management placement
