An exciting opportunity has arisen in the Older Persons Community Treatment Team Newcastle/North Tyneside, for a Band 6 Community Practitioner (Nurse, Social Worker or Allied Health Professional) based in the Centre For Aging and Vitality (CAV) on the old Newcastle General Hospital Site. We are seeking a highly motivated clinician to undertake assessment and offer treatments. Experience in working with older adults is required.
Job Responsibility:
Undertake assessment and offer treatments
Provide assessment and treatment function across a variety of environments e.g. community care home or patient's own accommodation
Act as a Care Coordinator working with the Mutli Disciplinary Team and also working with other agencies and voluntary sector
Have responsibility for a shared caseload working within and supported by a skilled and experienced MDT to deliver safe, effective, responsive and compassionate care
Provide care and interventions to service users with a range of complex mental health needs with associated risk
Requirements:
Professional Qualification and current Professional Registration as, a First Level Registered Nurse (RNMH, RMN, RNLD) or equivalent as an AHP, Social Worker or Clinical Psychologist
Evidence of post-registration training in clinical/ psychological interventions / skills relevant to the post
Up-to-date knowledge/experience of relevant legislation
Proven track record of post registration clinical experience relevant to the post
Mentor Training
12 to 24 month band 5 qualified nurse experience dependent upon service need
Awareness and understanding of professional accountability, NMC standards and other local and national initiatives
Proven ability to carry out and provide comprehensive written assessments of individuals with complex mental health needs
Risk Assessment and Risk Management / Safety Planning skills
Demonstrate effective written and verbal communication skills
Evidence of a high level of professional, organisational and leadership skills (self & others)
Excellent interpersonal skills
Ability to work autonomously
Ability to work effectively with other professionals using a multi-agency approach
Nice to have:
Post Registration Qualifications in Psychological Interventions
Experience of working within a community mental health setting
Knowledge of evidence based treatments for psychosis and non psychosis
Understanding of difficult to manage/challenging behaviours
