Are you a passionate and motivated individual, who is looking to further their knowledge in Health & Wellbeing? We have an exciting new role within our Integrated Wellbeing contract as a Smoking Cessation Practitioner. The post holder will be responsible for delivering evidence based, structured and tailored support to all smokers across Cheshire West & Chester at multiple locations and settings.
Job Responsibility:
Deliver evidence based, structured and tailored support to all smokers across Cheshire West & Chester at multiple locations and settings
Book and record information about interventions
Support all aspects of Health & Wellbeing delivery
Facilitate brief interventions on health & wellbeing topics
Signpost to additional support services
Attend and support delivery of outreach sessions and training
Work closely with wider team and other referring agencies
Create and deliver training sessions across Cheshire West & Chester
Dispensing and management of smoking cessation pharmacotherapy prescriptions
Organise and deliver face-to-face and group/drop in sessions across community spaces
Provide telephone-based support between 9am-8pm Mon-Fri
Manage own appointment bookings and clinic schedules
Ensure all data collected is kept up to date and support service audits
Engage with smokers and provide evidence-based motivational support
Provide compassionate interventions and signpost or refer people
Build and maintain good working relationships with community staff and organisations
Follow-up clients referred/signposted
Arrange and attend meetings, conferences, workshops and outreach events
Develop and maintain knowledge of health initiatives
Identify training needs within organisations
Deliver Smoking Cessation training programs
Devise new learning plans and resources
Conduct evaluations and feedback
Market available training courses
Work within allocated budgets
Ensure all data is entered into digital systems accurately
Ensure all data is processed securely and confidentially
Provide accurate accounts of interventions
Complete appropriate onward referrals
Provide feedback to referrers
Ensure all pharmacotherapy and equipment remains secure
Ensure Health & Safety requirements are met
Carry out any other relevant duties

Requirements:

Willingness to learn
Willingness to travel across Cheshire West & Chester
Confident public speaker
Ideally have experience in behaviour change or coaching
Good computer literacy and attention to detail
Ability and willingness to travel across the Cheshire West patch
Ability to work outside of normal working hours from time to time
