Job Description:

Join our team as a Band 6 Community Occupational Therapist and play a key role in supporting adults in the community to regain and maintain independence.

Job Responsibility:

Perform comprehensive patient assessments
Create and implement personalised care and rehabilitation plans
Deliver evidence-based interventions to optimise functional outcomes
Collaborate with multidisciplinary teams to provide holistic care
Maintain accurate, professional clinical records

Requirements:

A current and valid HCPC registration
Previous NHS or community-based experience, ideally with adults
Strong clinical reasoning, communication, and documentation skills
The ability to manage a varied caseload independently
A full UK driving licence and access to a vehicle (car driver essential)
A commitment to delivering high-quality, person-centred care
