Job Description:

Join our team as a Community Occupational Therapist and play a key role in improving the lives of adults living in the community.

Job Responsibility:

Perform comprehensive patient assessments in home and community settings
Create and implement personalised care and rehabilitation plans
Deliver evidence-based interventions to optimise independence and function
Collaborate with multidisciplinary teams to provide holistic care
Maintain accurate patient documentation and reports

Requirements:

A current and valid HCPC registration
Community-based occupational therapy experience
Strong communication and interpersonal skills
The ability to work autonomously and within a multidisciplinary team
Access to a car for community visits is essential
A commitment to delivering high-quality, person-centred care
