Job Responsibility

Assess and treat patients in community settings including private homes, care homes, and GP practices
Manage patients with neurological conditions, including stroke, head injury, Parkinson’s disease, MS, and other progressive disorders
Deliver high-quality rehabilitation and support for long-term condition management
Plan and carry out community visits across the Sutton borough
Work as part of an MDT, collaborating with GPs, nurses, physiotherapists, and other healthcare professionals
Maintain up-to-date and accurate patient records
