Job Description:

An exciting opportunity is available within The Scarborough Integrated Community team. We are looking for the right person who has a keen interest in Adult Mental health to join our wonderful team. This candidate will adhere to the Trust Values daily, work alongside patients, carers, families, external agencies and colleagues from a variety of disciplines. You will be a valued member of the team and will develop excellent working relationships with our partners and stakeholders. Job Responsibility:

  • Provide a service to those individuals who present with Mental Health issues, along with other additional needs/ risks but with an emphasis on supporting/ delivering psychologically informed brief interventions
  • Holding a case load of patients
  • Administering depot medications
  • Carrying out initial assessments
  • Duty and attending triage
  • Work collaboratively with service users of varying ages and differing needs as well as their carers and families
  • Manage complex individuals in partnership with other agencies providing care coordination
  • Commitment to the development of high quality care, and to engage in quality improvement activities

Requirements:

  • Current professional registration with Nursing & Midwifery Council RNMH or RNLD
  • To be recognised practice assessor / mentor or work towards in an agreed timescale
  • Trained Clinical Supervisor (within 6 month probationary period)
  • Evidence of Continuing Professional Development
  • Working with people with mental ill health / learning disabilities
  • Working in a person centred therapeutic organisation
  • Working collaboratively with service users and their families/carers
  • Working in a multi-disciplinary team
  • Working in partnership with other agencies
  • Evidence based knowledge of best practice in caring for patients in the designated field
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
  • Understanding of Safeguarding and its application in practice
  • Clinical Risk Assessment and Management and its application in practice
  • Clinical Governance and its application in practice
  • Confident working knowledge of clinical medicines management, including administration of injections
  • Ability to travel independently in accordance with Trust policies and service need
