Invicta Health in partnership with Kent and Medway NHS and Social Care Partnership Trust (KMPT) are seeking to recruit Community Mental Health Practitioners to be part of Mental Health Together. Mental Health Together is a new multi-agency service in Kent and Medway that brings services and agencies together to support people with varying mental health needs, ensuring they get the right care without navigating service boundaries.
Job Responsibility:
Foster recovery through empowerment of service users and carers using a strengths-based approach
Offer a range of evidence-based interventions and risk management approaches
Be proactive, innovative, and creative in delivering care
Engage service users and carers in relapse prevention and risk management plans
Liaise with inpatient units to support discharge
Facilitate early discharge
Deliver a flexible and responsive service
Work collaboratively with the multi-disciplinary team and other agencies
Deliver high quality care and provide solutions and signposting
Provide care within the Strength Based Model
Act as an educator for service users and carers
Respond swiftly to urgent referrals
Work closely with carers
Recognise racial, cultural, sexual preference and address health inequalities
Ensure record keeping/documentation reflects needs
Maximize the use of resources
Be responsible for assessing, planning, implementing, and evaluating care
Work within the MDT framework
Ensure care plans are reviewed and updated
Undertake risk assessment/management
Ensure activity/contact is entered promptly on RiO
Provide care plans in accordance with National Guidelines
Participate in the development of an effective team
Actively promote integrated health professional working
Facilitate the development of a positive and supportive team culture
Take an active interest in working parties and groups
Participate in the audit process
Advise, encourage and share knowledge
Challenge and raise concerns around poor or unprofessional practice
Participate in the induction of new staff
Participate and review staff performance
Act as a positive role model
Have a working knowledge of current appropriate legislations
Engage in regular clinical and professional supervision
Adhere to professional code of conduct
Maintain registration
Ensure physical health checks are carried out
Be accountable for own practice
Advocate and promote self-advocacy
Communicate effectively with the MDT
Ensure sensitive information is handled professionally
Respond to emergency situations
Act as a mentor to students
Ensure students are supported
Ensure own continued professional development
Undertake mandatory training
Undertake a regular appraisal
Support new staff
Support training and implementation of new policies
Requirements:
Minimum of 2 years post registration experience
Previous experience of working in a community team
Registered Mental Health Nurse, or registered Occupational Therapist (holding diploma or degree qualification)
MELPS (Mentoring and Enabling in a clinical setting)
