You will have the time and support to make a real difference every day. Come and join the Redcar & Cleveland community team. The Community Hub team are looking for a Mental Health Practitioner to join the team to aid our journey to change and improve the local community's experience with mental health services, adopting a 'no wrong door approach'. The Community Hub CMHT will support with short term interventions in collaboration with partner agencies and voluntary partners.

Job Responsibility:

  • Provide a service to those individuals who present with mental health issues, with an emphasis on psychologically informed brief interventions
  • Work collaboratively with service users of varying ages and differing needs as well as their carers and families
  • Deliver clinical interventions specific to care pathways
  • Manage complex cases in partnership with other agencies providing care coordination in line with CPA
  • Develop skills in psychological therapies, psychosocial interventions and care management
  • Encourage ongoing engagement from all staff utilising effective leadership skills
  • Use own initiative and make decisions independently
  • Commit to the development of high quality care, and to engage in quality improvement activities

Requirements:

  • Current professional registration with an approved professional body
  • Evidence of Continuing Professional Development
  • Key skills in literacy, numeracy and ITQ level 2 (or equivalent)
  • For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education or willingness to work towards
  • Trained Clinical Supervisor (within agreed timescale)
  • Working with people with mental ill health
  • Demonstrable knowledge to post-graduate level of evidence-based practice in caring for patients in the designated field
  • Understanding of relevant legislation (e.g. Mental Health Act, Mental Capacity Act)
  • Detailed understanding of Safeguarding and its application in practice
  • Clinical Risk Assessment and Management and its application in practice
  • Provide leadership and monitor, co-ordinate and prioritise the activities of a team
  • Communicate complex and sensitive information effectively to patients, carers/families and all members of the multidisciplinary team
  • Work effectively as part of a multidisciplinary team and undertake lead professional / care co-ordinator responsibilities
  • Provide effective clinical supervision, teaching, training and assessing in clinical practice
  • Able to work in accordance with the Staff Compact and Trust Values and Behaviours
  • Compassionate in meeting the needs of vulnerable people and their families and carers
  • Able to engage with vulnerable people and work effectively in distressing and challenging circumstances
  • Able to work flexibly and co-operatively as part of a team
