The Community Living Worker holds a critical direct support role within CLUOV, working under the direction of their supervisor and in collaboration with other team members to support people with disabilities in their community. This support entails, for example, facilitating relationships, managing finances, developing skills, attending appointments, assisting with personal care, working towards individualized goals, and establishing social roles that will promote the participation of people supported in a typical and valued life of the community.
Job Responsibility:
Maintains an in-depth knowledge of each person supported and their required support/needs
Participates in planning meetings, contributing to the Individual Support Plan and best support practices for people supported
Assists people supported to achieve identified goals as outlined in plans or otherwise established
Provides information and skill development that enables people supported to make informed decisions and choices
Facilitates and/or accompanies people supported to medical, dental, and other professional appointments as required
Dispenses, records, and signs for the administration of medication to people supported as per the orders of a health professional
Provides personal care such as lifting, transferring, toileting, incontinent care/changing, bathing, grooming, dressing, and other personal hygiene practices
Carries out household management duties required, such as laundry, dishes, cleaning, and tidying
Facilitate or complete meal planning, preparation, feeding, and associated clean up as required
Assists, when necessary, with the creation of and adherence to budgets, management of personal finances
Provides transportation to people supported as required
Maintains and promotes a safe, enhancing environment for people supported
Assists people supported to establish non-paid, natural relationships resulting in increased social capital
Pursues resources available in the community and liaises with groups, employers, and members to promote participation of people supported in professional, recreational, and social activities
Requirements:
Post-secondary education in the field of Human Services (i.e. Developmental Services Worker, Social Service Worker, Recreation and Leisure, Addictions, Psychology, Nursing)
or some combination of relevant education and/or experience
Must maintain a current and valid G or G2 Ontario Driver’s License with proof of at least $2 million vehicle insurance coverage
Provides a recent, clear Criminal Record and Vulnerable Sector Screening Check and Standard or Emergency First Aid/CPR Certificate
Acquires CPI Training Certification, Controlled Acts Training, Safe Food Handling Certificate, and other courses/training as required
Demonstrates superior oral and written communication skills
Uses analytical skills to collect, organize, analyze, and disseminate significant amounts of information with an attention to detail and accuracy
Is self-directed, maintain superior organizational and time management skills
Applies common sense and understanding to carry out instructions
Demonstrates the ability to take initiative, develop creative problem-solving solutions, form interpersonal relationships, and utilize collaboration and resource management skills
