Job Description:

Coordinates health care system outreach and education for the community. Administers programs to develop community relationships with healthcare providers. Drives health service awareness to assist members in maintaining optimal health outcomes.

Job Responsibility:

Coordinates health care system outreach and education for the community
Administers programs to develop community relationships with healthcare providers
Drives health service awareness to assist members in maintaining optimal health outcomes
Executes both routine and non-routine business support tasks for the Community Health area under limited supervision
Builds and maintains effective working relationships with members, providers, case managers, and agency representatives, and assists with care plan implementation
Manages initiatives to reduce culture and socio-economic barriers between members and providers and identifies gaps and opportunities to strengthen systems of care with the community
Maintains member health records, compiles reports, develops educational materials, and completes program documentation

Requirements:

Working knowledge of problem solving and decision making skills
High school diploma or equivalent required
Valid KY CHW certification
1+ years experience as CHW
