Job Description:

The role of a Community Health Worker at Oak Street Health involves removing barriers and increasing wellness across all life domains by providing culturally competent care, facilitating access to resources, and supporting patient advocacy. Responsibilities include proactive outreach, coordination of care plans, and high-quality service delivery to improve health outcomes.

Job Responsibility:

Establish and maintain strong interpersonal relationships with patients, community organizations, team members, and partners to coordinate patient needs
manage patient referrals defined by the care team and collaborate with the Medical Social Worker
facilitate communication between all identified parties involved in patients’ care
form relationships with and build an inventory of local community organizations
connect patients to state and local community resources
assist patients with completion of applications for accessing eligible benefits and resources
promote goal setting and achievement to improve patients’ quality of life and self-efficacy
meet with patients in patient-centered and patient-preferred locations
drive engagement with high-risk individuals
complete referrals to organizations and agencies
deliver culturally appropriate health education
support care team decision making through interdisciplinary team meetings
document interactions in the electronic medical record
manage time, set priorities, work independently, and collaborate effectively with the interdisciplinary team
