In collaboration with various internal and external stakeholders, this position is responsible for both coordination of and performing direct service activities designed to reduce the burden of asthma in Milwaukee County. A grant-funded position, the specific program outcomes and objectives will be determined annually in partnership with State of Wisconsin. Program components will be inclusive of but not limited to: community outreach and partnership, client and family health education, and outcome collection and analysis. The Asthma Management Program Coordinator provides a combination of direct services that includes in home asthma education, home environmental assessments, and small group classroom instruction within designated schools.

Requirements:
Graduate of an accredited school of nursing with BSN preferred OR Graduate of an accredited Respiratory Therapist Program with minimum of AS or AAS degree, BS preferred
A current RN license to practice professional nursing in the state of Wisconsin OR certification by National Board for Respiratory Care and license to practice respiratory care in the state of Wisconsin
Current CPR certification - Basic Life Support (BLS) for Healthcare Providers through American Heart or American Red Cross is required
Two years of nursing experience or respiratory care experience preferred with an emphasis on health education
previous experience in pediatrics preferred
Requires a valid driver's license, acceptable driving record, personal auto liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier
Must have reliable vehicle that can be used for conducting home visits and which may include transportation of supplies
A high level of communication skills (oral and written) necessary for assessment of health concerns and referral for appropriate medical advice and to effectively interact with school staff, public health staff, primary or specialty care providers, and other community agencies
Experience working with families of diverse ethnic, cultural and socioeconomic backgrounds strongly preferred.
