This role is a vital part of a targeted health inequalities project aimed at improving access to preventative health care for people with learning disabilities. You’ll work directly with individuals and families while collaborating with Primary Care, community organisations, and public health partners.
Job Responsibility:
Work directly with individuals with a learning disability, their families, and carers to promote awareness and understanding of Annual Health Checks
Deliver inclusive sessions in community venues, day services, support hubs, and public spaces
Provide one-to-one support and peer-informed outreach to build trust and increase confidence in using health services
Co-design accessible promotional materials (including Easy Read, visuals, and digital content)
Promote AHCs through outreach events, local media, social media, and community champions
Ensure messaging is tailored to meet the cultural and communication needs of diverse communities
Build and maintain strong working relationships with Primary Care stakeholders, statutory agencies, and Voluntary, community and faith-based organisations
Facilitate aligned working and shared campaigns to ensure consistent and inclusive messaging
Work closely with the project Steering Group, participating in regular meetings and using their insights to shape project delivery
Ensure people with lived experience have genuine input into the project’s planning, materials, and evaluation
Act as a conduit for feedback between the Steering Group and wider community voices
Record engagement activity, participant data, and feedback using agreed tools
Collect real-life stories, examples, and case studies to demonstrate impact
Contribute to written reports, dashboards, and presentations for funders, partners, and internal use
Support evaluation and reflective learning to inform future delivery
Requirements:
Experience of working with people with a learning disability or additional needs
Strong communication and interpersonal skills with an ability to engage across diverse communities
Experience of partnership working and/or collaborative project delivery
Knowledge of health inequalities and an understanding of barriers to healthcare access
Ability to deliver accessible outreach using a range of tools and formats
Commitment to inclusive practice, co-production, and empowerment
Car driver, with access to own vehicle
Nice to have:
Experience in primary care, social prescribing or community health
Knowledge of the Annual Health Check system
Lived experience of disability or caring for someone with additional needs
Experience working in or alongside Primary Care, social prescribing, or community health
Knowledge of Annual Health Checks and the UK primary care system
Experience of working with or supporting a project steering group or advisory panel
