Job Description

The Community Connection Advisor supports children, youth, and families by building strong connections to community resources and services. Using a person‑centered, culturally inclusive, and trauma‑informed approach, the Advisor builds networks of supports and services for children, youth, and families by using a person-centered approach and by applying the values and practices of community development. The Community Connection Advisor receives referrals and meets with families to identify assets and gifts of children and youth with support needs. Further referrals, supports, guidance, and problem-solving strategies to address issues related to family challenges, including reducing barriers to access services, are all part of the role.

Job Responsibility

  • Providing resources for families, developing advocacy skills, focusing on self-determination for children and youth with support needs and their families
  • Identifying assets and gifts of children and youth with support needs, taking direction from them and their families as to how they would like to connect and participate in the community
  • Participate in development of service plans and collaborate with team members to monitor progress and evaluate the effectiveness of recommended activities and need for modification as necessary
  • Assist families to complete applications, travel, and access to services by reducing barriers to access programs and support services
  • Prepare and conduct group educational and parenting sessions related to families’ needs
  • Complete the required documentation, including statistics, monthly and annual reports in a timely manner

Requirements

  • Diploma or certificate in an area of human and/or social services
  • One (1) year of experience working with children and families
  • An equivalent combination of education, training, and experience
  • Strong skills for effective communication – listening, speaking, and writing
  • Ability to use standard office technology and ability to learn new programs and applications
  • Strong Interpersonal skills to work with people of all ages and cultural backgrounds
  • Understands, demonstrates, and maintains professional boundaries with co-workers, clients and community partners
  • Clear Criminal Record Review
  • Physical ability to meet job requirements
  • Class 5 driver's license, clear driver's abstract and daily access to and use of vehicle
